Q&A: Can I get pregnant without a period?

by Dr. Gerard M. DiLeo

I'm 42 this January, have had 3 abortions, the last one with my husband in 1997. We didn't think we would have children then decided we'd like to try. I stopped birth control pills in August.

I had a positive pregnancy blood test at the hospital in early October (LMP Sept 18th), skipped a period as expected, then started bleeding heavily on what would be my 28th day of the 2nd month if I counted and skipped October. With strings of blood and clots, on November 13th. I would have been about 8 weeks gestational. The ER doctor suggested that it was a blighted ovum. I did not have a D & C as I bled for about 10 days (my normal period is about 5) so I assumed it was over and done with.

It has now been about 36 days since the first day of my "LMP" - heavy bleeding (which I assumed was basically a normal period) and although I normally have about a 31 day cycle I'm wondering when and if I'm going to get my period at all this month.

I had that egg white cervical mucus at about the 14th day this month, and my husband and I tried again. I tested with home pregnancy twice at 28 and 31 days this month. Negative.

Can I still get pregnant without a period? Or do I have to wait until I have normal cycles, with period ? If so, how many would you suggest it will take until the possibility of ovulation and pregnancy is on the horizon? How many more days should I wait for my period before I see my OB/GYN? I'm taking Celexa and Xanax so I want to be sure what's happening so that I'll quit, at least the Xanax right away.

I think you should see your OB-GYN as soon as possible so as to come up with a plan. It's unclear as to what really happened, what with the irregular bleeding, previous pregnancy, blighted ovum, etc. If you were a patient of mine, the first thing I would do is get a quantitative HCG to document resolution of the other pregnany event, then get ultrasound to check your uterus for any residual tissue. Re-establishment of regular cycles is helpful in managing a subsequent pregnancy, but you can get pregnant with a "last" period if you ovulate first and conceive. Yours is a complicated history and you need to have help with your OB on this one.
