Due to a decrease in fetal movement, I recently had a nonstress test done which found the baby's heartrate to slow down from 160 to 110 after movement.This was usually preceeded by a slight acceleration or shoulder. The NP decided to do a biophysical profile which looked good except there were no breathing movements noted during this time. They scored the biophysical profile a 6 out of 8. I seem more concerned about this then they are. Also, I should mention I am carrying twins with one demise at around 21 weeks. I am currently 30 weeks and want to know is this as normal as they imply and should I be looking for another opinion?

Assuming you're talking about THIS pregnancy having one viable twin and one demise, it wouldn't be unreasonable to ask that a maternal-fetal specialist supervise, review, or perform the biophysical profile. If you're talking about a previous demise in a previous pregnancy but are carrying twins now, you're still high risk from both past and present history.
The bio profile is the best test you can have short of a contraction stress test. The heart rate pattern isn't troubling if the bio profile looks good. As far as breathing movements go, the most common reason for not seeing it is not waiting long enough to see it. If it was done over about 10 minutes, that might not have been enough; if over 30-40 minutes, then this might bear a repeat bio profile within a week. Either way, wanting a 10 out of 10 on the biophysical profile isn't unreasonable when you've been through the trauma of a demise.
