I am 21 weeks pregnant and the past few days when I go to the bathroom, I have been having a clear discharge with my bowel movements. Sometimes just with gas. I only notice it in the toilet. Is this something I should call my doctor about? I have a cerclage and have been on bed rest for the past two and a half months.

Discharge with bowel movement can be vaginal secretions which are pushed out at the time of defecation, or it can be actually from the large intestine, and come out with the stool. I am assuming that you are certain that this discharge is vaginal, so I will answer the question in that way.
I don't think the fact that this happens with going to the bathroom really makes this different than just noticing that you have increased vaginal secretions. Vaginal secretions in mid-pregnancy could be the normal discharge of pregnancy (usually more noticeable with a cerclage), a vaginal infection, infection at the cerclage stitch, or even your mucus plug or amniotic fluid>.
If you have any clear fluid like you broke your water, or even one drop of blood, you need to be seen emergently. Anyone with a cerclage should feel free to call her doctor with questions, since the cerclage was provided because of concerns about your risk of preterm birth. They will know the specifics of your clinical situation and can give you personalized medical advice.
